What is spinal traction (back and neck)?

Traction is a physical therapy technique that reduces pressure on the spine or joints. A controlled pulling force is applied to help bring the bones, joints and muscles back into balance, relieve muscle spasm, reduce pain caused by pressure on the spine and restore the body’s movement. There are two types of traction:

1. Cervical traction, or traction of the neck: used to relieve neck pain
2. Lumbar traction, or traction of the lower back: used to reduce pressure on the lower back

Benefits of traction

  • Relieves back and neck pain by reducing pressure on the intervertebral discs and nerves
  • Increases spinal flexibility so the joints move better
  • Reduces pressure on the spinal nerves by widening the space between the vertebrae
  • Reduces pressure on the discs and reduces disc protrusion
  • Improves blood circulation, helping injured tissue recover faster
  • Promotes spinal alignment and reduces postural problems
  • Stretches the muscles to reduce muscle spasm

Who is traction suitable for?

  • Conditions caused by nerve root compression from a herniated disc
  • Spinal osteoarthritis (spondylosis) with pain or muscle spasm in the neck or back
  • Tight muscles or restricted joint movement
  • Mild lumbosacral spondylolisthesis (grade I–II)

Contraindications to traction

  • Infection or inflammation of the bones or intervertebral discs
  • People with bone cancer or spinal tumours
  • People with a ruptured disc or severe spinal injury
  • People with an unstable fracture or dislocation
  • People with osteoporosis or a risk of fractures
  • People with uncontrolled high blood pressure or heart disease
  • Pregnant women, especially in the third trimester
  • People with severe nerve compression causing loss of muscle, bowel or bladder control

Always tell your doctor and physical therapist about any underlying conditions, history of accidents or previous surgery before every treatment.

Frequently asked questions about traction

Q: Does traction hurt?
A: In general, traction should not cause pain. If you feel pain or discomfort, tell your physical therapist so they can adjust the pulling force.

Q: How many sessions will I need to see results?
A: The length of treatment depends on each individual’s condition. You may typically need 2–3 sessions per week, and it may take several weeks to see clear results. Combining traction with other physical therapy modalities may produce results more quickly, depending on your level of pain and your physical therapist’s advice.

Q: Does traction have any side effects?
A: Possible side effects include mild aches or dizziness after treatment. However, if you notice anything unusual, consult your physical therapist immediately.

Q: Is traction suitable for everyone?
A: Not everyone can have traction. You should be assessed by a doctor and physical therapist first, especially if you have any of the contraindications listed above.

Traction is a physical therapy technique that helps reduce pain, rebalance the spine and speed up recovery. However, it is not suitable for everyone, and you should be assessed by a doctor and physical therapist first for safe and effective treatment.
