Neck pain spreading to the head: migraine or muscle tension?

Neck pain that radiates up into the head or temples is so common that many people assume it is a migraine. In fact, the cause may actually be tense shoulder muscles.

How can we tell a headache caused by muscle tension from a migraine? First, it helps to understand that a migraine is caused by the arteries in the brain constricting and dilating more than normal. It is often triggered by stress or lack of sleep, and causes headaches on one side of the head, alternating sides, or on both sides. There are usually triggers such as colours, sounds, light or heat.

Pain that comes from shoulder muscles so tense that it radiates up into the head is usually found in people who use their shoulder muscles a great deal or sit and work for long periods all day, such as office workers, or people who repeatedly use poor postures. This makes the shoulder muscles tense up again and again until tender spots, known as trigger points, develop, which can cause pain radiating to the head or temples.

Self-care for people who are starting to have symptoms

1. Apply a warm compress to the shoulders and the back of the neck for 20 minutes at a time to relax the muscles around the shoulders and neck.

2. Stretch the shoulder muscles. To stretch the right shoulder, put your right arm behind your back, then tilt your neck to the left, using your left hand to help with the stretch. Stretch until you feel tightness but no pain, and hold for 15 seconds. Do 10 repetitions per set, 3 sets a day, alternating sides.

If your symptoms do not improve after trying these steps, we recommend seeing a physical therapist for treatment before the problem becomes more chronic. Physical therapy focuses on relaxing the muscles, releasing the painful spots, and exercises to improve muscle flexibility and strength, so that the muscles can cope with daily use and the pain does not come back.
